We view the grantmaking process to be a partnership. Our best funding relationships are those where we receive clear communication—on what’s going well, what’s not going well, and how we can help.

From the outset of the grantmaking process, we work with our non-profit partners to define the overall results we hope to achieve and the data needed to measure those results.

At an established date during the grant funding period, an Interim Evaluation Report on the progress of the project is required to be emailed to the Trustees. This date will be established at the time the grant is funded.

At the conclusion of the grant, a Final Evaluation Report is required to be emailed to the Trustees within 30 days of the completion of the project for an organization to be eligible for future grants.

Grantees should complete the Interim and Final Report Forms (below). Please email completed reports to Bethany M. Springer at: bethany.springer@usbank.com.

GRANT RECIPIENT INTERIM  EVALUATION REPORT
Due within 7 days after the reporting period

  • Name of organization

  • Contact person & title

  • Address

  • City, State, Zip

  • Phone number

  • E-mail address

  • Program name

  • Grant amount

  • Number of years program has been in existence

  • Number of children served by program during grant period

  • Brief summary of program (not to exceed three sentences)

  • Summary of program results: (Please do more than just express results in terms of the number, or increases in the number of clients served. We would expect that any program funded would include established goals at the start of the program to allow for a measure of success at the conclusion of the program. Those are the results to be discussed here).

  • Describe how the program could have been improved to more effectively deliver the intended service. (It is unacceptable to conclude that the program was without defect and could not be made more effective. That's not the real world. We expect that a vital component of any program is a continuous critique of the effectiveness of the program. Those are the results to be discussed here).

  • Brief description of future plans for this project/ program (operational and financial)

  • Provide a budget breakdown showing the uses of the grant during the immediate prior period
